What are the types of Thoracic Outlet Syndrome?

Entenda os tipos de desfiladeiro torácico

There are different causes and types of thoracic outlet syndrome.

Of the nerves

Neurogenic thoracic outlet syndrome occurs when the nerves that run from the neck to the arm (the brachial plexus) are compressed. More than 90% of cases are neurogenic.

From the vein

Thoracic outlet syndrome occurs when a vein is compressed, which can lead to thrombosis; 5% of cases are venous.

From the artery

The symptoms of thoracic outlet syndrome vary depending on the type of thoracic outlet. They also vary in intensity. Patients who work more with their hands for long periods experience more pain in their back and arms. They also report tingling, especially at night. Stressful days and long work hours make their nights a nightmare.

Sintomas da Síndrome do Desfiladeiro Torácico Neurogênico (dos nervos)

The main symptoms of neurogenic thoracic outlet syndrome (affecting the nerves) are pain or weakness in the arm and hand, tingling or discomfort in the fingers, arm that tires quickly, atrophy, and weakness in the hand.

Sintomas da Síndrome do Desfiladeiro Torácico Venoso

The main symptoms of thoracic outlet syndrome include swelling of the arm, hand, and fingers; painful tingling in the hand and arm; and very prominent veins, especially during flare-ups.

Sintomas da Síndrome do Desfiladeiro Torácico Arterial

Symptoms of thoracic outlet syndrome include cold and pale hands, pain in the hand and arm, especially during overhead arm movements, embolism (blockage) of an artery, and aneurysm of the subclavian artery.

What are the causes of Thoracic Outlet Syndrome?

Between 1% and 3% of the population has a cervical rib, which can grow on one side or both. Having this anatomical variation increases the chance of compression of nerves or blood vessels that pass through this small space between muscles and bones.

Músculo anormal ou formação da primeira costela

Abnormal muscle or first rib formation: some people may have an extra or aberrant scalene muscle (an internal neck muscle) or an abnormal first rib or clavicle. These deformities can compress blood vessels or nerves.

Traumatic injury

Many patients report that their symptoms began after an accident, mainly a whiplash injury (from a sudden car stop).

Tumor no pescoço

In rare cases, a tumor in the neck can cause nerve compression, resulting in thoracic outlet syndrome.

How is the diagnosis made?

The diagnosis of thoracic outlet syndrome is made through a physical and clinical examination of the patient, using provocative maneuvers to check for the presence of symptoms. Another important assessment is through imaging tests such as MRI and ultrasound.

Treatments for Thoracic Outlet Syndrome

Treatment for thoracic outlet syndrome is determined according to the specific syndrome affecting the patient—whether it is thoracic outlet syndrome, neurogenic, or vascular—and its degree of complexity.

Tratamento para Síndrome do desfiladeiro neurogênico

Treatment for neurogenic outlet syndrome commonly begins with physical therapy. If physical therapy does not alleviate the symptoms, microsurgery may become necessary.

Treatment for Thoracic Outlet Syndrome

Treatment for thoracic outlet syndrome initially involves rehabilitation. If initial treatment does not yield positive results, the patient may undergo microsurgery recommended for TOS.

Treatment for Thoracic Outlet Syndrome

Treatment for arterial thoracic outlet syndrome may include medication (anticoagulants) and reconstruction or replacement of the artery if it has an aneurysm or contains a clot. Surgery is recommended for arterial TOS in severe or refractory cases.

Bilateral treatment
